Don’t forget to bring your current insurance card(s) with you, so that we can assist in determining your benefits. We will accept co-insurance and co-pays at the time of your appointment and bill your insurance directly.

For HMO patients, please assist our billing team by obtaining a referral from your primary care physican prior to the scheduled appointment. Without this, your ppointment may be delayed.
